Refer a Patient for MSK Imaging & Injections

We accept referrals from GPs, physiotherapists, consultants and other clinicians. Patients are seen by Dr Prashant Sankaye, Consultant MSK Radiologist, at LSRI Harley Street (W1G 9QJ) or LSRI Northwest London in Harrow (HA2 7SJ). Use the form below, or call us to discuss a referral.

What to include

• The patient's name, date of birth and contact number
• The clinical question and relevant history, including red flags
• What you are requesting: MRI, ultrasound, X-ray, ultrasound-guided injection or consultation, and the body area
• Any previous imaging, with where and when it was done
• Insurer and authorisation number, if the patient is insured
• How urgent the referral is, and your contact details for the report
