Expert answers to common questions about tendon injections
Tendons have a poor blood supply compared to muscles, making them slower to heal. Injecting directly into a weight-bearing tendon with steroids can weaken it and increase rupture risk. Therefore, we use specialized techniques like PRP (to stimulate healing), high volume injections (to strip neovessels), or inject steroids only around the tendon sheath (tenosynovitis) rather than into the tendon substance itself.
High Volume Image Guided Injection involves injecting a large volume of saline and local anesthetic (sometimes with a small amount of steroid) between the tendon and the underlying fat pad. This mechanically strips away the small new blood vessels and nerve endings that grow into a degenerated tendon, reducing pain and allowing rehabilitation.
For chronic tendon degeneration (tendinopathy), PRP is often superior to steroids because it stimulates long-term tissue repair rather than just temporarily masking pain. Steroids can inhibit healing in chronic cases. PRP is particularly effective for tennis elbow, Achilles tendinopathy, and patellar tendinitis.
Tendon injections can be more uncomfortable than joint injections due to the sensitivity of the inflamed tissue. We use generous local anesthesia to numb the area first. You may feel pressure or a stinging sensation. Afterward, the area may be sore for a few days as the healing process initiates.
Yes, you can bear weight, but we strongly recommend wearing a walking boot or heel lifts for a few days to offload the tendon, especially after PRP or high volume injections. You should act as if you have a bad sprain and avoid any running or jumping for several weeks.
Tendon healing is slow. For steroid injections (sheath), relief is often quick (days). For PRP or regenerative treatments, pain may initially increase before improving over 4-6 weeks. Full structural healing and return to sport can take 3-6 months depending on the chronicity of the injury.
Absolutely. "Pills and needles don't strengthen tendons—exercise does." The injection opens a window of pain relief to allow you to perform loading exercises. Without progressive strengthening (rehabilitation), the tendon pain is likely to return. We work closely with physiotherapists to guide your recovery.
Calcific tendinitis involves calcium deposits forming within a tendon (often the rotator cuff), causing severe pain. We perform "barbotage" (lavage), where we use ultrasound guidance to puncture and suck out the calcium deposits with a needle while injecting saline. This provides rapid and significant relief for most patients.
